Alexander House recognised for its digital excellence

Alexander House has been recognised for being in the top 5% of digitally enabled care homes in South West London.

This home was awarded a certificate by the regional NHS Integrated Care Board (ICB), following the implementation of a number of digital projects including the Universal Care Plan (UCP).

The UCP, an NHS service, facilitates the digital sharing of personal data to healthcare professionals, to improve outcomes for patients. The platform provides easily accessible patient information including historical records, improving relationship building and coordination of care across services.

Andrea Howell-Jones, Registered Manager at Alexander House said:

“It’s been great to access the UCP and we’ve really noticed the benefits. It’s provided us with one key platform to refer to which has all of the information we require about a patient, from details of what they’d like to eat, to resuscitation preferences. It’s saving us time when liaising with healthcare professionals as they can see all of the relevant patient information too – we don’t have to spend extra time relaying these details.”

The Home has also implemented other digital projects, one of which is the Hydration Pilot. Residents have been supported in developing personalised hydration plans to help increase their fluid intake and reduce their risk of developing urinary tract infections (UTIs). Camascope, a medicine management software, is also helping to streamline medication management in the Home, by supporting deliveries and repeat prescriptions.
